Prevalence and risk factors for extended-spectrum β-lactamaseproducing Gram-negative bacterial infections in hospitalized patients at a tertiary care hospital, southwest Nigeria

Abstract:
Background: Clinical infections caused by extended-spectrum β-lactamase (ESBL)-producing bacteria constitute great burden to healthcare delivery with these resistant pathogens contributing largely to the magnitude and spread of antimicrobial resistance globally. Hence, knowledge of the risk factors for acquisition of infection caused by ESBL-producing bacteria is crucial to instituting prompt and appropriate treatment as well as prevention and control measures. This study investigated the risk factors associated with the prevalence of ESBL-producing Gram-negative bacteria (GNB) infections among hospitalized patients in Uniosun Teaching Hospital (UTH), Osogbo, Nigeria.

Methodology: A total of 359 hospitalized patients with clinical infections from whose clinical samples we isolated non-duplicate GNB were consecutively recruited. GNB were isolated following aerobic cultures of
appropriate clinical samples and MicrobactTMGNB 24E kit was used for species identification. All isolates were screened for ESBL production by the combination disc method. Relevant clinical and demographic information
was obtained using a designed data collection form, and multivariate logistic regression analysis was used to identify associated risk factors. Occurrence of unusual non-fermentative gram negative bacilli in intensive care units of a university hospital, Egypt

Abstract

Non-fermentative Gram-negative bacilli (NFGNB) other than Pseudomonas and Acinetobacter species have emerged as nosocomial pathogens. No much data is currently available concerning the occurrence of these types of bacteria in Zagazig University Hospitals (ZUHs). In this study, the occurrence as well as the antimicrobial susceptibility pattern of unusual NFGNB obtained from clinical samples collected from intensive care units (ICUs) of ZUHs was assessed. Additionally, the genetic relatedness among the most prevalent unusual NFGNB species was studied.

Results: Out of 516 non-repeated clinical sample, 97 NFGNB (18.7%) were isolated. Among them, 17 unusual NFGNB were identified by API 20NE, accounting for 17.5% of NFGNB and 3.3% of all tested samples. Within the unusual NFGNB, Burkholderiacepaciacomplex (Bcc) was the most prevalent species accounting for 94.1% of NFGNB and 3.1% of total samples. This was followed by Burkholderia pseudomallei (B. pseudomallei) which accounted for 5.9% of NFGNB and 0.2% of all obtained specimens. Tigecycline antibiotic was the most effective antibiotic against Bcc isolates (68.8% susceptibility) in disc diffusion method. After random amplified polymorphic DNA (RAPD) testing, the obtained Bcc isolates were found to be genetically diverse. This highlights Bcc as an emerging nosocomial pathogen in ICUs of ZUHs. Continuous monitoring of the occurrence of Bcc in ICU as well as in other hospital wards is warranted.

Abstract

Background: Epidemiology of extended-Spectrum- β-lactamases has become worldwide, and our aim was to establish the prevalence of isolates producer in university hospital center Yalgado OUEDRAOGO particularly in reanimation and emergencies units.

Material and methods: Prospective study was drive during July 2009 to march 2012 in order to collect strains resisting to third generation of cephalosporin during diagnosis analysis of biological specimens. Susceptibility of bacteria to antimicrobial agents was evaluated by disc diffusion method. Production of extended-spectrum β-lactamases has been investigated by double disc diffusion and kinetic methods.

Results: 259 isolates which resisted at least to one of third generation of cephalosporins were collected. Among them 188 (72, 58 %) were positive to synergy test by a double disc diffusion method. The MICs of ceftriaxone determined by E-test were under than 50kg/ml, 100kg/ml et 256kg/ml for respect 81,57°/° ; 55,26°/° et 39,74°/° of isolates. Hydrolyze of β-lactam ring by bacterial extract followed at spectrophotometer showed speeds running at 0 to 0,090UAb.mn-1 for both isolates. Extract of 171 bacterial strains positives to synergy test had hydrolyzed at least one of oxy-iminocephalosporins and were identified as producing extended- spectrum β-lactamases. Spices reported by this study were 99 Escherichia coli (57,89%) ; 28 Klebsisella pneumonia (16,37%) ; 15 Enterobactersp (8,77%) ; 19 Pseudomonas aeruginosa (11,11%) ; 4 Citrobactersp (2,33%) 2 Acinetobactersp (1,16%), 3 Proteus mirabilis (1,75%) and 1 Salmonella typhi (0,05%).

Conclusion: This study showed that bacterial resistances by extended- spectrum β-lactamases are a reality in University Hospital center YalgadoOuedraogo. It calls about antibiotics prescription and hospital hygiene in order to reduce emergence and propagation of new resisting bacterial.
