Bloodstream infection caused by Kocuria species in a tertiary hospital in Lagos, Nigeria: A case series

Abstract

Kocuria species are emerging opportunistic pathogens commonly misidentified as coagulase-negative staphylo- coccus. Improvement in laboratory diagnostic techniques and the expansion in the number of patients with immunocompromised diseases has led to the rising number of Kocuria infections being reported. We report cases of Kocuria bacteraemia in three immunocompromised patients. The first case was an 11-year-old with metastatic nasopharyngeal carcinoma and Kocuria kristinae isolated from the second blood culture, but due to discharge of the patient against medical advice, the role of K. kristinae and outcome of the case could not be ascertained. The second case was a 30-year-old with acute lymphoblastic leukaemia, with fever and diarrhoea, and Kocuria species isolated from the second blood culture, but despite treatment with antibiotic with invitro activity against the organism, the outcome was fatal. The third case was a very low birth weight premature male neonate who was delivered by emergency cesarean at 31 weeks gestation, due to maternal chronic hypertension and superimposed preeclampsia. Although, Kocuria rosea was isolated from the third blood culture of the case, it was most likely a colonizer, as the clinical condition of the patient had improved before isolating the organism. This report may be the first case of bacteraemia caused by Kocuria from Nigeria, indicating that this organism should not ignored as a possible cause of bacteraemia because advancement in laboratory diagnostics will aid correct identification. Abstract:

Background: Acute otitis media (AOM) is one of the most common bacterial infections in children. The emergence of antimicrobial resistance (AMR) poses a threat to the management of these infections. The objective of this study was to identify and describe the sensitivity profile of bacterial agents isolated in AOM to commonly used antibiotics in the city of Ouagadougou, Burkina Faso.                           

Methodology: This was a descriptive cross-sectional study conducted between September 2023 and February 2024 in children aged 0-15 years with clinical features of suppurative AOM seen at the otorhinolaryngology (ORL) departments of four health facilities in Ouagadougou. Ear discharge was collected using sterile swabs and cultured on Chocolate agar media, incubated at 37°C for 24-48 hours in 5% CO₂. The sensitivity of the isolated bacteria to antibiotics was determined using the Kirby-Bauer disc diffusion method and interpreted according to the 2021 guideline of the Antibiogram Committee of the French Society of Microbiology (CA-SFM).

Results: A total of 61 children with AOM were recruited and bacteriological culture examination was performed on the ear swabs obtained from them. All but two cultures were positive. A total of 59 bacterial pathogens were isolated with Gram-negative bacilli (GNB) more commonly isolated (67.8%, n=40) in the children, dominated by Klebsiella pneumoniae (18.6%, n=11) and Pseudomonas aeruginosa (18.6%, n=11). Aztreonam and imipenem were the most active antibiotics against GNB, but majority of the GNB were sensitive to gentamicin and ciprofloxacin.                          

Conclusion: Acute otitis media in children, combined with the growing resistance of pathogens to antibiotics, is a real public health challenge. It is necessary to strengthen regular microbiological monitoring and the development of appropriate therapeutic strategies in order to limit the emergence and spread of resistant strains.

Antibiotic resistance profile of Non-Fermenting Gramnegative bacteria recovered from clinical specimens in a tertiary care teaching hospital, Hyderabad, India

 Abstract

 Background: Non-Fermenting Gram-negative bacilli (NFGNB), including Pseudomonas aeruginosa, Acinetobacter species, and Burkholderia species, are emerging as significant opportunistic pathogens, especially in immunocompromised and hospitalized patients. These organisms pose a major therapeutic challenge due to their multidrug-resistance (MDR) and intrinsic resistance mechanisms. The goal of this study is to determine antibiotic susceptibility pattern of NFGNB isolated from clinical specimens of patients in a tertiary care teaching hospital in Hyderabad, India.

Methodology: A descriptive cross-sectional observational study of patients with clinical infections was conducted between December 2024 and April 2025, to identify NFGNB from their clinical specimens. Different clinical samples (blood, sputum, urine, pus and others) were collected from patients of both sexes and all age groups for bacterial isolation using automated Bact/Alert for blood culture and conventional method for other samples. Identification of NFGNB isolates and antimicrobial susceptibility testing (AST) of the isolates against selected antibiotics were performed using the VITEK 2 Compact system, and AST interpretations were made in line with the Clinical and Laboratory Standards Institute (CLSI) guidelines. Prevalence and predictors of hepatitis B screening among pregnant women attending antenatal clinics in three primary health centres in Abeokuta, Nigeria: A cross-sectional study

Abstract:

 Background: Hepatitis B virus (HBV) infection remains a major public‑health concern. Antenatal screening identifies maternal infection and enables interventions to prevent mother‑to‑child transmission. We assessed the prevalence and predictors of prior HBV screening among pregnant women attending three primary health centres in southwest Nigeria.

Methodology: This was a cross‑sectional survey of 289 pregnant women consecutively enrolled at the antenatal clinics of the three selected health centres. A standardized questionnaire was interviewer-administered to each participant to collect information on socio‑demographic characteristics, previous HBV screening and behavioral and healthcare‑ factors related to HBV screening. Data were analysed using SPSS. Associations of factors with prior HBV screening were determined using Chi‑square and multivariable logistic regression estimated adjusted Odd ratio (AOR) with 95% confidence interval (CI).
